You will need to copy this form and mail it.

There is a total of 4 Videos. The Anniversary Mass, the Anniversary Party in the hall,  The Congratulation Video, and the Photo show of his priesthood. We have a order form in the vestibule.

The videos will be $10.00 each or Set of all four for $30.00

DVD’s each is $10.00 or all 4 for $30.00 this includes shipping and handling. We will mail you the DVD’s or you may pick them up at the Church Office. If I know what Mass you will be at I will bring them to the Mass.
